Let's see if I understand the problem you're facing:

- You read a paper and annotate it in Zotero

- In Obsidian you create a literature note and now your have all your annotations for the paper

Now, you go back to that paper in Zotero and add highlights and notes. If I understand correctly you want your note in Obsidian to be updated with your new content.

This is not possible at the moment. The only workaround I can think of is that you delete your note and create a new literature note.

The button you talked about "Refresh Citation Database" is for when you just made some changes to the bib file and your Obsidian Citations plugin is not picking up the citations.

At the moment my workflow is that I highlight in Zotero and import to Obsidian once only. In your case, I think maybe taking notes from the paper could be separate from your highlights.

I'm sorry about this, however I know that some users are working on improved versions of this citations plugin. I'll update the article once they do. In the meantime, perhaps follow me on Twitter to hear about updates on the article.
